Become our Patient in 3 easy steps:
Step 1 - Call 803-607-9430 to verify we accept your insurance
Step 2 - Download and fill out application & medical history
Step 3 - Bring insurance card, ID and forms to Premier Peds

We will review your paperwork for completeness, copy and verify your insurance card and ID, and answer any questions you may have.
The approval process takes 3-5 days. Dr Buschor reviews and determines how best to care for your child. We will contact you with her decision.
